Hepatorenal acute kidney injury (Hepatorenal Syndrome)

Targeting Renal Perfusion in Hepatorenal Acute Kidney Injury (HR-AKI) in hospitalized cirrhotic subjects.

A prospective data collection at OMC (and other collaborative institutions) to test the hypothesis that an increase in MAP ≥ 15 mmHg or RPP ≥10 mmHg is associated with 35% improvement in rate of recovery of HR-AKI

Predictive value of diagnosis of acute on chronic liver disease (aCLD vs chronic stable ESLD (sESLD) in the response to vasoconstrictor therapy in HR-AKI.

A retrospective data collection. The hypothesis is that subjects with aCLD are more likely to respond to therapy.

Diagnostic and prognostic value of ASL MRI perfusion of the kidneys in HR-AKI.

The hypothesis is that ASL MRI can distinguish HRS from ischemic ATN and predict response to vasoconstrictor therapy.

Vancomycin precipitous acute kidney injury - Mechanism

Interference of tubular secretion of creatinine by vancomycin.

Cell culture-based study. In a cell line expressing key organic transporters (OATs), test transport of creatinine +/- vancomycin, dose dependence.

Incidence of precipitous AKI with high dose vancomycin in obese subjects.

A prospective data collection. The hypothesis is that obese patients treated with > 2 g per day of IV vancomycin are at higher risk of precipitous AKI.
