  • Queensland Health - The CHSRI Contract with HIU is to progress shared clinical priorities, innovative models of care and integrated advice. This involves evidence-based advice and leadership in clinical governance, and the role of clinical networking across primary and secondary care to improve patient experience, health outcomes and efficiency.

  • International Foundation for Integrated Care (IFIC) - The Centre is an academic partner with the International Foundation for Integrated Care, based in the UK, and an organizational member of IFIC Australia.  The collaboration will support 3 upcoming international meetings: the IFIC Summer School July 3-7 in Oxford, the 1st Asia-Pacific Conference on Integrated Care Nov 6-8 2017 in Brisbane and the 4th International Health Care Reform Conference 21-23 March 2018 in Sydney.
  • Coordinare Primary Health Network, New South Wales - Coordinare is partnering with the Centre to develop an approach for the application of the Primary Care Practice Improvement Tool (PC-PIT) Process as part of Primary Health Network quality improvement programs. This includes the use of the PC-PIT in supporting practices to transition to Health Care Home Models of care. The Centre and Coordinare are working together to develop a framework to guide the use of the PC-PIT in PHNs nationwide.
  • Capital Health Network, ACT - Capital Health Network is a Primary Health Network partnering with the Centre to trial the use of the PC-PIT Process as part of their existing quality improvement and practice support programs. This includes the use of the PC-PIT in supporting practices to transition to Health Care Home Models of care. The Centre and Coordinare are working together to develop a framework to guide the use of the  PC-PIT in PHNs nationwide.
