As a UQ medical student you are required to complete a number of compulsory annual program requirements. Some of the associated tasks can be done well in advance, while others cannot be completed until your enrolment becomes active, or you are on campus in Brisbane.

It is important that you read the information below carefully and ensure that you provide all documentation by the posted deadlines. If you have any queries, please do not hesitate to contact the student administration team (

2. Student Immunisation Record

MD Students are required to provide evidence of their immunisation and blood-borne virus status. The immunisation list is aligned to the Australian Immunisation Handbook and Queensland Health policy.

When to complete this task:

MD Students are required to provide a student immunisation record at the commencement of their program (first year).

Later year students will only be required to provide a new immunisation record if their BBV status changes, if their immunisation to pertussis lapses, or if vaccination requirements change.

It is highly recommended that you complete this form, in collaboration with your medical practitioner (doctor/GP), as soon as possible after accepting your offer into the MD program. This will ensure that you have enough time to complete any additional vaccinations that are required and to take steps to confirm immunity if needed. The form must be submitted to the Faculty of Medicine as soon as possible and no later than 17 February 2020. 

If your BBV status makes you ineligible to engage in exposure-prone procedures (Section 3 of the form) please contact the Manager of Student Administration as soon as possible ( 

Step by step instructions to complete this task:

  1. Print out the Student Immunisation form
  2. Work with a registered medical practitioner to complete the immunisation form, including documenting immunity and reviewing evidence of your BBV status.
  3. Read the Tuberculosis (TB) Screening Information and complete the Queensland Health Tuberculosis Risk Assessment Form for Students.
  4. Upload documents to Blackboard

Please note the following important details:

  • All sections of the form must be completed;
  • A registered medical practitioner must complete Section 2, 3 and 4 in the spaces provided on the form.  There is no need to upload any laboratory documents except where requested on the form. e.g. hepatitis B immunity (titre level).
