As a UQ medical student you are required to complete a number of compulsory annual program requirements. Some of the associated tasks can be done well in advance, while others cannot be completed until your enrolment becomes active, or you are on campus in Brisbane.

It is important that you read the information below carefully and ensure that you provide all documentation by the posted deadlines. If you have any queries, please do not hesitate to contact the student administration team ( 

Unless otherwise stated all forms must be submitted via Blackboard. Please refer to Section 8 of this page for important information about this portal and how to navigate it.


1. Blue Card

Trainee students who work with children and young people in Queensland must hold a valid blue card if their work falls into a category of regulated employment, regardless of how often they will come into contact with children and young people. It is the student’s responsibility to ensure that they have a valid Blue Card at all times.

If your blue card expires and you have not submitted your renewal application at least 30 days before the expiry date, you cannot continue in your studies in the clinical environment until after you have received a new blue card.

When to complete this task:

Students are required to bring the completed form in person during Orientation week of Year 1. The Faculty of Medicine is unable to accept or process this form prior to Orientation week.

Step by step instructions to complete this task:

New Blue Card Application
  1. Print out the prefilled Blue Card Application Form and complete Part D (Applicant Details).
  2. Ensure that you have two current original identification documents that meet the requirements of Part E (Proof of Identity) that you can bring to Orientation.
  3. During Orientation Week submit the completed application form to the Faculty of Medicine office (Level 4, Building 69, St Lucia campus) who will sight the identification documents. Faculty of Medicine staff will process and submit the form on behalf of all students to Blue Card Services.
  4. As a guide, it takes at least 28 business days for your application to be processed. If you have not received any correspondence during this time, please contact Blue Card Services directly.
  5. After you have received your physical copy of the blue card, you must keep it with you at all times.
Existing Blue Card Holders
  1. Print out the prefilled Link and applicant/cardholder to this organisation form and complete Part A (Applicant Details), Part C (Current application/card details) and Part F (Cardholder/application declaration).
  2. During Orientation week submit the completed application form to a Faculty of Medicine Professional staff member. Faculty of Medicine staff will process and submit the form on behalf of all students to Blue Card Services. Please make sure to bring your current valid Blue card which will need to be sighted at time of submission.
  3. Keep a copy of your current Blue Card with you at all times.
Blue Card Renewals

You can submit paperwork for Blue Card renewal at your assigned Clinical Unit, or the Faculty offices at St Lucia and Herston at least 30 days before the expiry date.
